Well it does. You can choose from the short straight shooter, or the ribbed attachment the sprays in a circular patten (think sprinkler). Both do the job well. I think the attachment is more comfortable. It very easy to use and clean. After you have pooped, fill it with warm water insert and squirt. Hold then expel, repeat as required. Don't forget to clean the douche when you have finished.

Decent sized bulb holds plenty of water and works just fine with the thin nozzle, but the ribbed attachment makes an otherwise clinical cleansing a bit more fun. The rounded tip is much more comfortable to insert and the ribs on the nozzle feel great, without being too wide. It makes for a great warm up to anal play. Easy to pull apart for cleaning.
